PARSA Treatment Works Approval (TWA) Checklist

When applying to PARSA for the endorsement of a TWA the following information should be included for PARSA’s file:

NOTE: TWA Forms can be found at the NJDEP Bureau of Construction and Connection Permits Website--  http://www.state.nj.us/dep/dwq/forms_twa.htm

o   Copy of the TWA Application (Signed and sealed)

o   Copy of the Engineers Report Form WQM-006  (signed And sealed)

o   Original completed Consent Form WQM-003.   NOTE: Resolutions authorizing appropriate signature from Municipalities and Sewerage Authorities (as applicable) on ALL submitted forms (TWA-1, WQM-003) are required.   

o   Copy of appropriate USGS Quadrangle Map with project site clearly delineated

o   One (1) set of Final Plans and Profiles signed and sealed by a New Jersey Licensed Professional Engineer on each sheet. Plans must be marked "FINAL" or "FINAL FOR SEWER DESIGN". Plans are to include standard details of sewerage appurtenances.   NOTE: if a Pumping Station is included in the design, please include all technical information regarding it

o   Application Review Fee of $250, payable to PARSA   (NOTE:  additional; review fees may apply based on the complexity of the project)

Process

The time frame for review varies – if the project is less than 4500 gallons per day, or 15 units, the Executive Director is authorized to sign those without Board approval.  These are usually returned within 7 days.   If the project exceeds those limits, the PARSA Board of Commissioners will approve it at their monthly meeting (1st Thursday of the month).  This may take up to 45 days. PARSA will return the original WQM-003, with the appropriate signatures and authorizing resolution; all other paperwork supplied will be retained for the file.
